Accessing your visit notes
When your provider completes a visit, your follow-up notes and care instructions are made available through Elation Passport, a secure health records platform used by Direct2Care's clinical team. Setting up your Elation Passport account is a one-time process. After that, any future visit notes will appear there automatically.
Setting up your Elation Passport account
After your visit is completed, you'll receive two things:
- A text message containing an invitation code
- An email from Elation Passport with a link to get started
Note: The email will come from an Elation domain, not a Direct2Care address — this is expected. If you don't see it in your inbox, check your spam folder.
To access your visit notes for the first time:
- Open the email from Elation Passport and tap the link.
- Enter the invitation code from your text message when prompted.
- Complete the account setup steps to create your Elation Passport account.
- Once your account is created, your visit notes and care instructions will be available there.
This only needs to be done once. Future visit notes from any completed Direct2Care visit will appear in the same Elation Passport account automatically.
Accessing notes from future visits
After your initial setup, log into your Elation Passport account at elationpassport.com or through the Elation Passport app, available on the App Store (iOS only).
Reviewing your visit in the Direct2Care app
You can also review your visit details and intake responses directly in the Direct2Care app at any time.
- Tap the Visits tab in the navigation bar.
- Tap the completed visit you'd like to review.
- On the visit details screen, you'll see the visit status, location, price, organization, plan, and intake language.
- Tap Review Intake to see the responses you provided during intake, including your age verification and consent to treat confirmation.
Note: The Review Intake option is available for all visits regardless of status.
