Getting a doctor's note
Direct2Care providers can issue notes for work, school, or any other purpose that requires documentation of your visit. There's no additional cost, it's included with your visit.
How to request one
Simply let your provider know during your visit that you need a note. They'll prepare it before the visit ends.
Your note will be available through your Elation Passport account after your visit is completed, along with your visit summary and care instructions. See [Accessing your visit notes] if you haven't set up your Elation Passport account yet.
Notes for a child's visit
If you need a note for a child's school or doctor's appointment, request it the same way, let the provider know during the visit. Make sure the child is added to your account as a household member beforehand. See [Managing household members] for help with that.
